Register Early: Working dog positions are limited and will be reserved on a first paid basis. Refunds will be issued only if another participant is found to replace a reserved working position.

Lunch: There are no restaurants or fast food places near by. You are welcome to bring your own lunch, or lunch (main course, salad, dessert and beverage) will be provided for $5.00 per day.  Lunch reservations and payment must be made in advance to facilitate meal planning and preparation.

EVENT AND GUEST RELEASE AND WAIVER OF LIABILITY

MICHAEL ELLIS SEMINAR

WHEREAS, Penn Ohio Working Dog Club is holding a training event on November 15 and 16, 2008;

WHEREAS, I, the undersigned, have voluntarily chosen to attend said event;

NOW THEREFORE, I hereby waive, release and discharge any and all claims for damages for death, personal injury, and/ or property damage which I, or my heirs, may have, or which may subsequently accrue to me or my heirs, as a result of or arising from my participation in any subject training event.  This Release and Waiver is intended to, and shall operate to, discharge in advance the promoters, sponsors, officials, land owners, and other participants from and against any and all liability arising out of or connected in any way with my participation in this event, even though that liability may arise out of negligence or carelessness on the part of any person or entity above mentioned.

I FURTHER STATE THAT, I am aware various breeds of dogs may be present at this training event or seminar; I am familiar with the nature of this training event; I am aware that injury and damage can occur during this event or seminar, both to myself and any dog(s) I may have.  Knowing the risks, I hereby agree to assume those risks and to release and hold harmless all of the persons or entities mentioned above who might otherwise be liable to me or my heirs and assigns for damages.

I UNDERSTAND that no animals are allowed at the event or seminar without express prior permission by application to said event or seminar and that I assume all liabilities associated with the presence of any animal owned by me at said event.  All animals are expected to be under control by the owner.  A failure to control any animal will result in the ejection of such animal from the event, without refund.

I HAVE READ AND AM IN AGREEMENT WITH THE TERMS OF THIS RELEASE AND UNDERSTAND THAT BY SIGNING THIS RELEASE I AM RELINQUISHING AND WAIVING CERTAIN LEGAL RIGHTS I OTHERWISE WOULD HAVE.
